Will you accept my insurance and what is your policy about payment?

 

The Center for TMJ and Sleep Apnea is dedicated to making sure you achieve your overall health goals. However, we cannot allow insurance companies to dictate your treatment or your health outcomes. Our goal is to ensure that we deliver the level of care you deserve.

Therefore, we do not participate with any medical plans as a preferred provider. We are considered out-of-network.

As a courtesy to you, we will provide you with proper documentation to file your claim at your third in-office visit. If your insurance has out-of-network benefits, they may reimburse you directly for any services provided. Services provided could be applied towards your out-of-network deductible by your insurance carrier.

Any questions regarding pre-authorization/pre-determination should be directed to the insurance carrier by you, the patient.
