This page explains how Pain and Sleep Therapy Center of Charlotte ("we," "us," "our") collects and uses information through this website. Our website is a marketing site only. It does not collect protected health information (PHI). Patient health information is handled separately in our clinical systems under HIPAA-aligned practices.
Information we collect on this website
When you fill out the contact form, you provide your name, email address, phone number, preferred contact method, a free-text reason for your visit, and (optionally) how you heard about us. We expressly ask that you not include diagnoses, medications, insurance numbers, or dates of birth in this form.
Like most websites, we collect basic analytics. Pages viewed, approximate location (city/region), device type, and referring URL. Via Google Analytics. Analytics data is aggregated and is not linked to your identity by us.
How we use your information
We use the information you submit to respond to your inquiry and follow up about scheduling. Form submissions are forwarded to our practice management system. We do not sell or rent your information.

This site uses cookies for site functionality and analytics. You can disable cookies in your browser settings; some site functionality may be reduced as a result.
Third parties
We use third-party services to operate this website. Hosting (Netlify), analytics (Google Analytics), and form delivery to our CRM. These providers process information on our behalf under their own privacy practices.
Text messaging (SMS)
By checking the consent box on our contact form, you agree to receive calls, text messages, and emails from Pain and Sleep Therapy Center of Charlotte regarding your inquiry, scheduling, and care coordination. Message frequency varies. Message and data rates may apply. Reply STOP to opt out of text messages at any time, or HELP for assistance. Consent is not a condition of receiving care.
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, excluding vendors and service providers acting on our behalf to deliver those messages.

Information submitted through this website is transmitted over encrypted connections (HTTPS/TLS). Form submissions are delivered to our practice management system, where access is limited to authorized staff. We retain website inquiry information only as long as needed to respond and coordinate care, and we review our security practices periodically.

You may request that we delete any non-clinical information we hold about you by contacting us at contact@pscharlotte.com. Clinical records are governed by HIPAA and our practice's Notice of Privacy Practices, which is provided separately at your first visit.
